Thanks for the reply, I think this could be the problem. Although, I can't seem to figure out how to clear the memory on a mongrel, do you know how to do this?

We've had instances where a mongrel doesn't properly restart on a
deploy and an older version of the app ends up getting hit (from
memory) whenever that mongrel handles a request. If you're getting a
weird, intermittent error from a single server I'd investigate whether
or not something like that is going on. Especially if you're running,
say, 3 application instances and it's happening a third of the time.
